This file is a LiveCycle Designer form. Many non-Adobe PDF viewers will have serious problems processing such a file correctly, so you should make sure you only open it using Adobe Acrobat or Adobe Reader, and not directly inside a browser window, as suggested above.
Also, this file is not going to work if you try to open it on a mobile device. You have to use a desktop computer to open it.
